NO PURCHASE NECESSARY: Find “Land O’Frost Premium® / Mad Money Mondays” Game (The “Game”) game pieces inserted in specially-marked packages of Land O’Frost Premium® products from July 12, 2010 through September 20, 2010, or while supplies last, whichever occurs first. Land O’Frost, Inc. (“Sponsor”) is not responsible for missing or damaged game pieces and assumes no liability for retail packages not containing game pieces. Game may also be played without making a purchase by entering through the mail For mail-in entry, hand-print your name, mailing address, date of birth and daytime telephone number on a 3” x 5” piece of paper, insert the paper into an envelope with sufficient postage affixed and mail it to “Land O’Frost / Mad Money Mondays Game Entry”; Henson Consulting, Inc., Attn: Katie Cosgrove, 111 W. Wesley Ave, Suite 5, Wheaton, IL 60187 postmarked by September 20, 2010 and received by September 25, 2010. Sponsor will assign each valid mail-in entry a Game Code and include it in the appropriate weekly drawing. For purposes of this Game, a “Week” is defined as seven (7) consecutive calendar days beginning on a Monday and ending on a Sunday. Each online entry must contain a separate Game Code, and each mail-in entry must be sent in a separate outer stamped mailing envelope. Sponsor is not responsible for lost, late, misdirected, incomplete, illegible, damaged or postage-due entries. Liability for game piece containing printing or other errors is limited to replacement with another game piece, while supplies last. Material submitted become the property of the Sponsor and will not be returned.
  2. Eligibility: Participation open to legal residents of the fifty (50) United States and District of Columbia who are 18 years of age or older at the time of entry, except employees of Land O’Frost, Inc., its subsidiaries, affiliates, divisions, advertising and promotion agencies, individuals engaged in the development, production or distribution of materials for this Game, their immediate family members and persons living in the same households of each are not eligible to participate. Void where prohibited by law.
  3. How To Play: Each “Land O’Frost Premium® / Mad Money Mondays” Game piece has a printed game code (“Game Code”) appearing on the gaming panel. To participate in the Game online, visit the following URL address between 9:59a.m. ET on July 12, 2010 and 11:59p.m. ET on September 20, 2010 (Promotion Period”): www.landomoms.com. Click on the special “Land O’Frost Premium® / Mad Money Mondays Game” icon to be redirected to the gaming page. Follow the on-screen instructions on the gaming page to register for the Game and to submit the Game Code appearing on your game piece. Online entry limited to game pieces with valid Game Codes.
  4. Prizes & Retail Value: Ten (10) Grand Prizes (one prize per Week): Grand Prize is $1,000 awarded in the form of a check. Retail value of each Grand Prize is $1,000. Fifty (50) First Prizes (five prizes per week): First Prize is $500 awarded in the form of a check. Retail value of each First Prize is $500. One-Hundred Fifty (150) Second Prizes (fifteen prizes per week): $100 awarded in the form of a check. Retail value of each Second Prize is $100. Total Retail Value of all available prizes is $50,000.00.
  5. Game Drawings / Selection of Winners: Ten (10) Weekly random drawings will be conducted according to the Weekly drawing schedule in the chart below from among all valid entries (online and mail-in) submitted during the appropriate Weekly entry period. For mail-in entries, the “Submission Date” is the postmark date on the outer stamped mailing envelope. All decisions shall be final and binding in all respects. Odds of winning a prize depend upon the total number of valid entries received for each given Submission period. Non-winning weekly entries will not roll over into any remaining drawings.

    Potential Prize winners will be notified by email, phone and/or mail on or about five (5) days after the Weekly drawing. Return of any prize notification or prize mailing as undeliverable will result in disqualification. All potential winners are subject to verification before any prize will be awarded. In the event that any potential winner is disqualified for any reason, an alternate winner will be selected in a random drawing from all eligible entries received during the respective Submission Period. Allow 6-8 weeks for prize fulfillment. Unclaimed prizes will not be awarded.

  6. Conditions of Participation: Entrants are solely responsible for participating in the Game during the Promotion Period and properly claiming a prize if selected as a winner. No prize substitution or transfer of prize prior to awarding. By participating, you agree to these Official Rules and the decisions of the judges which are final and binding in all respects, and to release Land O’Frost, Inc. and all other companies associated with the “Land O’Frost / Mad Money Mondays” Game from all claims or liability relating to your participation or acceptance or use of prizes. Neither Land O’Frost, Inc, nor its affiliates, subsidiaries, divisions, advertising or promotion agencies are responsible for incorrect game piece or Game Code printing or seeding. Acceptance of a prize shall be construed as and signify the winner’s agreement and consent that Sponsor may use the winner’s name, voice, likeness and/or prize information, without limitation, for promotional purposes without further consideration, review, approval or payment, where allowed by law. In no event will more than the stated number of prizes in Rule #4 be awarded. All Game materials subject to verification. Game materials are null and void and will be rejected if not obtained through authorized, legitimate channels or if any part is counterfeited, illegible, mutilated, or tampered with in any way. Grand Prize and First Prize winners will be required to sign and return an Affidavit of Eligibility/Liability & Publicity Release/Prize Acceptance Form within 15 days of first attempted notification. Noncompliance with this time period may result in disqualification and forfeiture of prize and an alternate may be selected. The reporting and payment of any tax liabilities, if any, incurred by a winner as a result of this Game and on any prize won is the sole responsibility of the winner. Grand Prize winners will receive a 1099 tax form for the approximate retail value of their prize.
  7. Internet: Sponsor not responsible for lost, late, mutilated or illegible entries nor for electronic transmission errors resulting in omission, interruption, deletion, defect, delay in operations or transmission, theft or destruction or unauthorized access to or alterations of entry materials, or for technical, network, telephone equipment, electronic, computer, hardware or software malfunctions or limitations of any kind, or inaccurate transmissions of or failure to receive entry information by Sponsor or presenter on account of technical problems or traffic congestion on the Internet or at any website or any combination thereof. If for any reason the Internet portion of the program is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Sponsor which corrupt or affect the administration, security, fairness, integrity, or proper conduct of this Game, the Sponsor reserves the right at its sole discretion, to cancel, terminate, modify or suspend the Internet portion of the Game and to award all remaining prizes via valid Internet entries received up to the point of the action taken by the Sponsor and all valid mail-in entries received during the Promotion Period . Caution: Any attempt by any person to deliberately damage any website or undermine the legitimate operation of the game is a violation of criminal and civil laws and should such an attempt be made, Sponsor reserve the right to seek damages from any such person to the fullest extent of the law.
